Abstract

Introduction: the laryngeal mask is a extraglottic device safe for handling ventilation and oxygenation of the patient, however it can produce complications such as laryngeal spasm that can trigger an acute pulmonary edema of negative.
Objective: To show an anesthesiology clinical course of a patient with acute pulmonary edema by negative pressure after removal of laryngeal mask.
Case description: Young patient with no medical history, whom he underwent an orthopedic surgical procedure under general anesthesia with laryngeal mask and mechanical ventilation; after removal of the laryngeal mask presented laryngospasm, which determined important inspiratory effort; 5 minutes later he developed progressive dyspnea hypoxemia and hemoptysis. It was perfomed tracheal intubation with rapid sequence induction; alveolar infiltrates were found in the chest radiograph bilateral. The management of this pulmonary complication was held in the ICU; 36 hours after were extubated according to satisfactory clinical evolution, without further complications.
Conclusion: the review of the literature emphasizes the importance of prevention and early detection of laryngeal spasm, to avoid the serious complications that can occur which is the pulmonary edema negative pressure which should be managed in an ICU.
