Abstract

Introduction: Giant hiatal hernia with intrathoracic stomach is a rare condition. Its clinical manifestations are nonspecific, apart from including vomiting and thoracic or epigastric pain. Complications such as volvulus, perforation or gastric obstruction can occur, being such cases of medical emergency. In many cases, they can be detected incidentally by means of a thoracic X-ray. Targeted imaging studies, particularly the computed tomography or the barium study of the esophagus, stomach and duodenum, are fundamental for the diagnosis and characterization of the anatomy of these lesions. Their treatment is surgical, either by open or laparoscopic surgery.

Objective: To describe the clinical signs, complementary studies, behavior and evolution of a case with a diagnosis of gastric necrosis due to intrathoracic hiatal hernia.

Case presentation: A case is presented of a 30-year-old female patient admitted to the hospital with abdominal and thoracic pain, requiring urgent surgery after a diagnosis of giant hiatal hernia with intrathoracic stomach, as well as organoaxial gastric rotation.

Conclusions: Hiatal hernia with intrathoracic stomach and herniation of other organs to the thorax, such as colon, omentum, liver, small intestine and pancreas, is a rare form of presentation for them. Imaging studies, particularly tomography and barium study, are a cornerstone in the diagnosis of this pathology and its complications.
